Abstract

Objective

The purpose of this study was to investigate quality of reporting acupuncture intervention for mild cognitive impairment (MCI) based on the STRICTA and CONSORT guidelines

Methods

We searched for randomized controlled trial studies of acupuncture treatment for MCI in the MEDLINE (PubMed) database. Once the online search was completed, studies were selected manually by the inclusion criteria, and the selected studies were evaluated by STRICTA and CONSORT guidelines.

Results

Five studies were included. The most frequently selected points for acupuncture treatment of MCI were on the head, such as GV20, EX-HN1, GV24, and GB20. By STRICTA guidelines, all items from STRICTA were reported except three items on the checklist. By CONSORT guidelines, apart from six items which were not available, quality assessment was performed with 31 items. Eighteen items were reported by 80% of the studies. Four items were reported by 40% of the studies and another four items were reported by 20% of the studies. One item was not reported in all studies.

Conclusion

Most items were reported by STRICTA guidelines, whereas many items were insufficiently reported based on CONSORT guidelines. This needs to be considered in future RCTs of acupuncture treatment for MCI.

I. 서 론

경도인지장애(Mild Cognitive Impairment, 이하 MCI)는 치매로 발전할 위험이 있는 임상 전 단계의 환자를 확인하기 위하여 정상 노인과 치매 환자 사이의 영역을 조사하면서 생겨난 개념이다1. 치매를 진단하고 치료하는 과정에서 MCI의 개념은 지난 10년 동안 상당한 신뢰성을 얻게 되었고 매년 약 12%의 MCI 환자가 알츠하이머병이나 다른 치매로 전환되고 있음이 확인되었다2. 따라서 치매로의 이행을 최대한 줄이기 위해 MCI의 치료 필요성이 증대되고 있는 실정이다.

MCI의 약물적인 치료법 중 아직까지 효과가 입증된 약물이 없는 것으로 보고되고 있어3 비약물적인 치료법이 많은 관심을 받고 있다4. 여러 무작위배정 대조 임상시험(Randomized controlled trials, 이하 RCT)과 실험연구(Experimental study)의 메타분석(meta-analyses)을 통해 인지재활훈련과 같은 비약물적인 치료법이 인지 기능에 효과가 있음이 밝혀지고 있어5, 최근에는 인지재활훈련을 포함한 다양한 인지중재치료 기법이 활용되고 있다6. 추가적으로 MCI에 대한 한의학적 치료 효과의 긍정적 인식과 함께 의⋅한협진이 제안되는 추세이다7. 하지만 아직까지 한의계 내에서 임상적으로 활용하고 있는 비약물치료의 대표적 한의치료 기술인 침 치료 효과에 대한 체계적 연구는 부족한 실정이다.

RCT 설계가 중재 효과를 평가하는데 최적으로 간주되지만8, RCT의 효과적인 설계에도 불구하고, 부적절한 연구방법은 결과의 신뢰성과 유효성에 영향을 미칠 수 있다. 이에 저자는 현재까지 국내에서 MCI의 침 치료에 대한 RCT 연구가 보고된 바 없기에 PubMed를 통해 관련 해외 논문을 검색하여 선별 후, STRICTA(STandards for Reporting Interventions in Clinical Trials of Acupuncture)와 CONSORT(Consolidated Standard of Report in Trials) 지침을 바탕으로 평가를 수행하여 향후 국내에서 관련 침 중재 임상 연구의 참고 자료로 제안하고자 한다.

II. 연구방법

1. 문헌 연구 대상 선정

1) 검색 데이터베이스 및 검색 방법

2019년 8월 15일에 PubMed(https://www.ncbi. nlm.nih.gov/pubmed)에서 mild cognitive impairment의 MeSH 용어로 cognition disorders, cognitive dysfunction, neurocognitive disorders, 침 치료의 MeSH 용어로 acupuncture, acupuncture therapy, acupuncture points를 사용해 검색하였다(Table 1).

Search Strategy

논문 발행 연도는 2010년 1월 1일부터 2019년 8월 15일까지 출판된 논문으로 제한하였다.

이렇게 검색된 논문들은 Send to를 활용하여 빠짐없이 포함시켰다.

2. 연구 방법

2명의 연구자들(SYO, HWY)이 검색된 논문들을 모두 수작업으로 선정 기준에 따라 평가하고 합의하여 분석할 논문들을 선정하였다.

1) 경도인지장애에 대한 침 치료 시행

2) 사람을 대상으로 하는 연구

3) RCT 연구

MCI의 침 치료에 대한 RCT 연구 논문에 대한 질 평가를 위해 사용한 CONSORT 지침은 해석에 따른 오차를 줄이기 위해 CONSORT 2010의 공식 한글번역본을 참고하였다9. STRICTA는 공식사이트에서 제시하고 있는 항목을 참고하였다10.

분석에 사용한 STRICTA와 CONSORT 지침의 세부 항목 평가 중에 의견이 상충되는 부분은 두 연구자가 논의를 통해 정하였다.

III. 결 과

1. 문헌 검색 결과

PubMed 검색을 통해 총 226편의 논문이 검색되었고, 제목을 확인하여 사람을 대상으로 한 연구가 아닌 99편, RCT가 아닌 75편, MCI와 관련이 없는 논문 47편이 제외되었다. 그리하여 남은 5편의 전문을 확인하여 적합성을 평가하고 최종적으로 5편을 포함시켰다(Fig. 1).

2. 분석 논문의 일반적 특성

각 논문의 저널 이름, 각 연구에서 사용된 대조군 및 치료군의 처치 방법, 참여자 수, 출판 국가에 대해 정리하였다(Table 2). 논문 5편의 임상 연구는 모두 중국에서 시행되었고, 5편 중 3편11-13은 중국의 국내 저널에, 2편14,15은 국제 저널에 투고하였다. MCI에 대한 침 치료 효과를 보기 위해 설계된 치료군의 처치 방법은 1편14의 논문을 제외한 4편11-13,15에서 모두 대조군의 처치 방법에 침 치료를 추가해 치료하는 방식이었다. Wang14 논문만이 침 치료를 시행하지 않은 대조군에 대해 오직 침 치료만 시행한 치료군을 두어 비교 연구하였다.

3. 경혈 빈도 분석

논문 5편의 임상 연구에서 사용한 혈자리를 분석하여 최소 3편의 논문에서 공통적으로 사용한 혈자리를 표시하였다(Fig. 2). 임상 연구에서 가장 많이 사용한 혈자리 순으로 백회(GV20)가 4편11-14, 사신총(EX-HN1)11,12,14, 태충(LR3)12,14,15, 신정(GV24)11-13이 각각 3편의 논문에서 사용되었다. 그 밖에 풍지(GB20)11,14, 풍륭(ST40)14,15이 각각 2편의 논문에서 사용되었다.

4. STRICTA 지침의 세부 항목에 대한 질 평가 결과

STRICTA 지침의 세부 항목 기준에 따라 5편의 논문을 평가하여 정리한 결과(Table 3), 모든 논문이 총 16개의 세부 항목 중 3개의 세부 항목 2a, 5, 7을 제외한 모든 항목에 대하여 충실히 보고하고 있었다. 항목 2a(환자 당 자침수)에 대해서 4편11-13,15에서 정확히 보고하지 않았다. 항목 5(시술자의 배경)에 대해서는 3편11-13에서 보고하지 않았는데, 3편 모두 연구가 시행된 기관까지만 기재하였고 시술자에 대한 정확한 설명이 없었다. 항목 7(이상반응)에 대해서는 1편12에서 보고하지 않았다.

5. CONSORT 지침의 세부 항목에 대한 질 평가 결과

각 5편의 논문을 CONSORT 지침의 각 세부 항목에 대해 평가하여 해당하는 내용이 기재되어 있는 논문의 페이지 번호와 보고한 논문을 전체 논문에 대한 퍼센트(%)율로 나타내었다(Table 4). CONSORT 지침의 세부 항목은 총 37개로 이 중 6개의 항목은 논문을 평가하기에는 해당 사항이 없는 것으로 판단되어 N/A(Not available)로 표기하였다.

N/A로 표기한 항목을 제외한 각 세부 항목별로 보고된 논문을 전체 논문에 대한 퍼센트(%)율로 나타낸 그래프(Fig. 3)를 보면 80% 이상의 보고율을 보인 항목은 18개 항목이었다.

각 세부 항목 중 40%(2편)만 보고된 항목들은 3a(배정비를 포함한 임상 시험 설계{예, 평행설계, 요인설계} 기술), 12b(소그룹 분석과 보정 분석과 같은 추가 분석의 방법), 20(임상 시험의 한계, 잠재적 비뚤림의 원인, 부정확성, 또 관련되는 경우, 분석의 다중성), 25(연구비 및 기타 지원{예. 약품 공급}의 출처, 연구비 지원자의 역할)로 국제 학술지 2편14,15에서만 보고하였다.

각 세부 항목 중 20%(1편)만 보고된 항목들은 10(누가 무작위 배정 순서를 생성하였는지, 누가 참가자들을 등록시켰는지, 누가 참가자들을 등록시켰는지, 누가 참가자들을 중재에 할당하였는지 기술), 11a(눈가림을 했다면, 중재에 배당된 후 누가 눈가림 대상이 되었는지{예. 참가자, 치료 제공자, 결과 평가자}또 어떻게 눈가림이 되었는지), 13b(각 군에 대해, 무작위 배정 이후 손실과 배제 및 이유), 18(소그룹 분석, 보정 분석 포함, 사전에 정한 분석과 탐색적 분석을 구분한 모든 분석의 결과) 항목이었다.

모든 논문에서 보고하지 않은 세부 항목은 24(가능하다면, 임상 시험의 프로토콜 전문을 어디에서 접근할 수 있는지) 항목이었다.

최종적으로 연구대상으로 선정된 5편의 논문을 STRICTA와 CONSORT 지침을 바탕으로 평가하여 5편 모두 충실하게 보고한 항목에 대해 정리해본 결과(Fig. 4), STRICTA의 전체 16개 세부 항목 중 13개 항목에서 충실히 보고하였고, CONSORT의 전체 31개 세부 항목 중 12개 항목에서 충실히 보고하였다.

IV. 고 찰

정상 노인 인구에서 매년 1-2%가 알츠하이머병으로 진행하는 것에 비해, MCI에서 10-15%가 알츠하이머병으로 진행되는 것으로 보고되고 있다16. 더불어 MCI에 약물요법과 비약물요법의 병용치료 시 약물요법 단일 치료보다 효과가 더 좋다는 연구 결과들이 보고되면서9 최근에는 비약물적인 치료로 한의 치료 기술인 침에 대한 긍정적 인식과 함께 의⋅한 협진이 제안되고 있다7. 그러나 아직까지 국내에서는 MCI의 침 치료 효과를 입증하기 위한 RCT 연구 논문이 없으며, 해외 RCT 연구 논문을 참고해야 하는 것이 현 실태이다.

이에 본 연구에서는 향후 국내에서 MCI의 침 치료에 대한 RCT 연구를 보고할 때 참고 자료가 될 만한 자료를 제시하고자 침 중재 임상 연구 논문 평가에 흔히 사용되는 STRICTA와 CONSORT 지침을 바탕으로 해외 RCT 임상 연구 논문을 평가하여 제시하였다. MCI의 침 치료에 대해 1984년 5월부터 2010년까지의 RCT 연구 논문들을 대상으로 한 질 평가 연구가 있으며17, 검색 데이터베이스를 CENTRAL, Pubmed, EMbase, MEDLINE, CINAL, CNKI, CBMdisc, VIP를 사용했고, 분석방법은 본 연구와 마찬가지로 CONSORT와 STRICTA로 평가를 하였다. Lu17 논문에서는 항목별 보고율만을 보고하였는데 대체로 충실히 보고되지 않았다는 결과를 보였다. 각 논문별 세부 항목에 대한 분석은 제시하지 않았고, 분석에 사용하지 않은 항목도 있는 것으로 보였다. 본 연구에서는 Pubmed 데이터베이스에서 2010년도부터 발행된 RCT 연구 논문들을 대상으로 각 논문별로 CONSORT와 STRICTA 지침의 세부 항목에 대한 분석 결과를 제시를 하였으며, 세부 항목별 보고율을 비교하였을 때 Lu17 논문에서 분석된 2010년 이전의 논문들에 비해 2010년 이후의 논문들에서 월등히 높은 수준으로 보고하고 있는 것으로 파악되었다.

최종적으로 연구대상으로 선정된 5편의 논문 모두에서 침 치료가 MCI에 효과가 있다는 결론을 보여주었다. 각 임상 연구에서 MCI의 침 치료에 사용한 혈자리들을 보면 백회(GV20), 사신총(EX-HN1), 신정(GV24), 풍지(GB20)로 주로 두경부에 위치하는 혈자리를 사용한 것을 알 수 있다.

STRICTA 점검표에 대한 분석에서는, 시술자의 배경과 이상 반응 항목을 제외한 항목들이 모든 논문에서 충실히 보고하는 것으로 나타났다. 보고가 미흡했던 자침 수 항목의 경우 보고하지 않은 논문 4편 중 3편12,13,15은 환자의 증상에 따라 혈자리를 가감하여 사용하였기에 환자 당 자침수가 통일되지 않아 보고하지 않은 것으로 보인다. 모두 누락한 항목은 5(시술자의 배경) 항목이었다. 이로 인해 수기가 중요한 침술에서 시술자간 잠재적 차이로 숙련도, 전문 지식에서 편차가 생길 가능성이 있다. 또한 이상반응과 관련한 항목은 1편12에서 누락하였다. 출혈, 통증, 감각이상 등 일시적 반응들도 치료효과에 미치는 영향을 고려하면 사소한 반응까지도 이상반응으로 반드시 보고하여야 한다. 더불어 어느 시술 부위에서 이상반응이 나타났는지 까지도 보고한다면 침 치료 효과 증가와 안전성 확보에 기여를 할 것이라 여겨진다.

CONSORT 점검표에 대한 분석에서 해당 사항이 없거나 침 치료라는 특성으로 인해 해당 사항이 없는 항목들은 3b(임상 시험 개시 이후의 중요한 방법상의 변화와 그 근거{예, 포함기준}), 6b(임상 시험 개시 이후 시험 성과에 영향을 미치는 모든 요소의 변화와 그 근거), 7b(해당되는 경우, 중간분석과 중단지침에 대한 설명), 11b(관련이 있다면, 중재의 유사성을 기술), 14b(임상 시험이 종료되거나 중단된 이유), 17b(이분형 변수의 결과에 대해, 절대, 상대 효과 크기를 모두 표시하도록 권장)이었다. 이에 남은 항목들을 대상으로 분석했을 때 STRICTA의 항목보다 적은 항목들이 보고된 것으로 나타났다. 누락되거나 불충분한 항목들은 구체적인 RCT 설계방법, 비뚤림을 최소화 하거나 무작위성을 부여하기 위한 항목들에 대한 것으로 나타났다. 이는 침 치료의 특성상 눈가림이나 다양한 무작위성을 포함하는 설계가 어려운 점이 그 이유라 사료된다. 중국 내 학술지에 게재된 논문 3편11-13의 경우, CONSORT 점검 항목 중 평균 17항목(전체 31항목 중 55%)이었으나, 국제학술지에 게재된 논문 2편14,15의 경우, 평균 28항목(전체 31항목 중 90%)이 보고되어 중국 내 학술지에 출판된 논문보다 국제학술지에 출판된 논문이 지침에 더 부합하였다.

STRICTA와 CONSORT를 비교해 볼 때 STRICTA가 침 치료 RCT 논문 평가를 위해 개발된 평가방법이지만 본 연구에서 CONSORT에서 누락된 항목들은 비뚤림 위험을 제거하여 재현성이나 해당 RCT의 중재 방법에 대한 순수한 효과를 측정할 수 있도록 고안된 항목이기 때문에 누락된 항목들을 간과할 수는 없을 것으로 사료된다. 따라서 이를 보완하기 위한 방법으로 침 치료 RCT연구에서 대조군에 Sham 침 치료나 환자가 해당 침 치료를 받았다는 느낌을 주는 처치가 필요할 것이며 나아가 비뚤림 위험을 최소화 시킬 수 있는 RCT에 준하는 침 치료 임상 시험 모델을 개발하는 것도 필요한 것으로 보인다. 또한 이 논문에서 분석한 논문은 총 5편으로 MCI에 대한 RCT연구 활동이 부족하다고 보이며 특히 모두 중국 논문이라는 점에서 국내의 해당 연구에 대한 필요성이 더욱 요구된다. 따라서 MCI에 대한 RCT연구는 국내에서 활발히 이루어질 필요성이 있다고 사료된다.

V. 결 론

총 5편의 임상 연구를 분석한 결과 MCI를 치료하기 위해 백회(GV20), 사신총(EX-HN1), 신정(GV24), 풍지(GB20)와 같은 두경부 혈자리가 다용되었다. STRICTA와 CONSORT 지침을 이용하여 분석한 결과, STRICTA에서는 시술자나 이상반응과 관련된 3개의 세부항목을 제외한 모든 항목에 대해 모든 논문이 보고한 반면 CONSORT에서는 임상시험설계, 무작위배정수행, 눈가림과 관련된 세부항목에서 보고가 미흡했다. 추후 국내에서 MCI 치료를 위한 침 중재 RCT 연구를 진행할 때 이러한 부분을 보완하여 진행할 필요가 있다.
