유방암 환자의 림프부종에 대한 한의학적 치료 : 체계적 문헌 고찰

Abstract

Objectives:

Traditional oriental medicine is used in treating breast cancer-related lymphedema to alleviate symptoms. Upper limb lymphedema is a symptom that is frequently observed in patients with breast cancer, and it impairs their quality of life. This systematic review aimed to summarize the current available evidence to evaluate the effect of traditional oriental medicine on upper limb lymphedema in breast cancer patients.

Methods:

The review evaluated randomized controlled trials (RCTs) measuring the effect of herbal medicine, acupuncture, and moxibustion on upper limb lymphedema in breast cancer patients within four electronic databases. The Cochrane risk of bias (ROB) tool was used to assess the quality of the RCTs.

Results:

In total, 23 RCTs met the inclusion criteria. Among them, 22 studies reported that the rate of severity of lymphedema improved after treatment in the traditional treatment group using herbal medicine, acupuncture, or moxibustion better than in the conventional medicine group. The methodological quality of the RCTs was insufficient with an unclear and high ROB.

Conclusions:

Traditional oriental medicine may have a potential to improve lymphedema in patients with breast cancer. To confirm the clinical recommendation, further research with a rigorous study design is required to support the effects of traditional oriental medicine.

I. 서 론

세계적으로 여성암 발생 빈도 중 1위를 차지하고 있는 유방암은 우리나라에서도 그 빈도가 증가하고 있다1,2. 최근의 통계에 의하면 국내 유방암 발병률은 꾸준히 증가추세를 보여 1999년 10만명 당 20.9명에서 2012년 10만명 당 44.7명까지 높아졌다3.
림프부종이란 혈관을 통과한 간질액의 양과 이를 이동시키는 림프관의 이동 능력 사이 불균형으로 인하여 조직에 액체와 단백질 등이 축적되는 경우를 말하며, 주로 사지에서 발생하지만 얼굴, 등, 배, 가슴, 성기 등 신체 어느 부위에서나 생길 수 있다4.
일차성 림프부종은 선천적으로 림프관 발달에 문제가 발생하는 것으로서 6,000명당 1명의 빈도로 발생하고, 이차성 림프부종은 림프관이나 림프절이 손상되어 림프액 이동경로에 문제가 생기는 것으로서 0.13~2% 정도로 추정된다. 특히 유방암, 자궁경부암, 난소암의 20% 이상에서, 전체 암환자의 15.5%에서 발생한다고 보고된 바 있다5.
상지 림프부종은 유방암 환자에서 치료 목적으로 시행하는 액와림프절 절제술 후 가장 흔하게 발생할 수 있는 합병증으로써, 환자들에게 기능적, 미용적, 정신적으로 문제를 일으켜 삶의 질을 저하시킬 수 있다고 여겨진다. 게다가 장기적으로 보았을 때, 상지 외의 부위에서도 합병증을 발생시킬 수 있다. 일반적으로, 유방암 치료를 받은 환자들 중 10~20%에서 상지 림프부종을 경험하고 있다6.
통상 의학은 유방암 환자의 림프부종을 해결하기 위해 약물요법, 수술요법, 운동요법 등을 사용한다. 감염 위험을 줄이기 위한 피부 관리, 유연성 운동, 수중 운동 등 림프부종을 감소시킬 수 있는 운동요법과 diosmin, rutin, hesperidin 등의 benzo- pyrones제제와 furosemide와 같은 이뇨제 또는 림프부종으로 인한 감염이나 합병증을 예방하기 위한 셀레니움 제제의 복용 등을 치료법으로 쓰기도 한다. 비수술적 방법으로 회복이 어려운 경우 계속 부종이 진행되어 3기 또는 4기의 림프부종으로 발전하기도 하는데 이러한 경우 림프-정맥 문합술이나 림프-림프 문합술 등의 림프액을 배액시키기 위한 시술이 사용되기도 한다7.
림프부종을 치료하는 통상의학적 치료가 다양하게 이뤄지고 있으나 그 한계점이 지적되면서, 전세계적으로 보완대체의학에 대한 관심이 높아지는 추세이다8. 이와 관련하여 국내외에서 한약, 침, 뜸을 이용한 유방암 환자의 림프부종 치료에 관한 연구가 이뤄지고 있다. 국내에서는 사암침법을 활용하여 유방암 환자의 부종을 완화시키고 삶의 질을 높여주는 방법에 대한 선행연구가 진행되었고9, 도침술과 정맥자락술 또는 오령산을 이용한 치험례 보고 연구도 진행된 바 있으며10,11국외에서도 운동치료와 키네시오테이핑과 더불어 침술을 림프부종 치료에 사용한 사례에 대한 고찰이 이루어졌다12.
현재까지 유방암 환자의 림프부종에 대한 한약치료, 침치료, 뜸치료만을 각각 단독으로 연구한 논문은 있으나 한의학적 치료를 전반적으로 고찰한 논문은 발표되지 않았다. 본 연구에서는 한의학적 치료가 유방암 환자의 림프부종에 미치는 임상적 효과에 대한 근거를 제공하기 위하여 기존의 연구들을 체계적으로 고찰하였다.

II. 연구대상 및 방법

1. 문헌선정기준

림프부종을 가진 유방암 환자를 대상으로 한약치료와 침치료, 뜸치료를 시행 한 후 그 효과를 연구한 국내외의 논문을 대상으로 하였다. 선정 기준은 다음과 같으며, 선정 기준에 부합하지 않은 연구는 제외하였다.
1) 연구대상 : 연령, 병기와 무관한 유방암 환자
2) 중 재
(1) 한약치료, 침치료, 뜸치료
(2) 투약 유형, 기간, 횟수에 제한을 두지 않음.
(3) 한방치료 단독 혹은 병용 치료를 포함.
3) 비교중재 : 통상 치료 혹은 무치료
4) 연구유형 : 무작위대조시험(randomized controlled trial, RCT)
5) 언 어 : 한국어, 영어, 중국어로 제한

2. 연구 방법

1) DB및 검색 방법

문헌검색을 위해 Pubmed, The Cochrane Central Register of Controlled Trials, Research Information Sharing Service(RISS), China National Knowledge Infrastructure(CNKI)를 Data Base(DB)로 이용하였으며, 2019년 4월 1일까지 검색하였다. 언어는 한국어, 영어, 중국어로 제한하였으며 출판된 시기에는 제한을 두지 않았다. 검색어로는 ‘acupuncture’ 또는 ‘moxibustion’ 또는 ‘herbal medicine’과 ‘breast cancer, lymphedema’를 사용하였다.

2) 문헌 선택 과정

2명의 연구자가 독립적으로 논문 제목과 초록을 확인하여 문헌을 1차 선별하였고, 전문을 확보하여 선정 기준에 부합하는 연구를 2차 선별하였다. 전 과정은 독립적으로 수행되었다. 두 연구자 간의 합의를 통하여 최종적으로 포함될 문헌을 선택하였고, 서로 의견이 불일치할 경우 제 3자가 개입하여 토의를 통해 다수결로 결정하는 과정을 거쳤다.

3) 비뚤림 위험 평가

최종적으로 포함된 문헌에 대한 비뚤림 위험 평가는 2명의 검토자가 독립적으로 시행하였으며, 2명의 연구자가 합의를 이루지 못한 경우 제 3자가 개입하여 상의를 거친 뒤 결정하였다. 연구의 질 평가 도구는 코크란 비뚤림 위험 평가 도구(the Cochrane’s Risk of Bias tool version 5.3.5)13를 이용하였고, 세부적인 판정 기준은 NECA 체계적 문헌고찰 매뉴얼14을 참조하였다.

III. 결 과

1. 문헌 선정

일차적으로 검색된 문헌에서 중복된 문헌을 제외하고, 문헌의 제목과 초록을 검토하여 선정 기준에 부합하지 않은 논문을 배제하였다. 남은 문헌은 전문을 확인하여 한의학적 치료과정이 명확히 언급되지 않은 연구, 림프부종 호전에 대한 결과가 없는 연구들을 배제하였다.
4개의 DB 검색 결과 1차로 검색된 문헌은 총 98편 이었으며, 이 중 중복되는 문헌으로 14편을 제외하였고, 제목과 초록을 검토하여 38편의 문헌을 제외하였으며, 전문을 검토하여 RCT가 아닌 연구, 한의학적 치료 과정이 명확히 언급되지 않은 연구, 림프부종 호전에 대한 결과가 없는 연구들을 배제하였다. 최종적으로 23편의 문헌이 선정 기준에 부합하여 본 체계적 고찰에 포함되었다.

2. 선정된 문헌의 일반적 특성

최종 선정된 23편의 문헌 중 22편이 중국에서 진행된 연구이며, 1편은 호주에서 진행된 연구15이다. 16편은 학술지에 게재된 논문이며 7편은 학위논문18,20,21,23,31,33.34이었다. 22편은 2-arm study로 한방치료와 통상치료의 병용군과 통상치료 단독군을 비교하였거나, 또는 한방치료 단독군과 통상치료 단독군을 비교하였다. 3-arm study는 1편으로 한약과 통상치료의 병용군과 한약 단독치료군, 통상치료 단독군을 비교하였다. 각 연구의 표본수의 범위는 30~130명이었다. 탈락률을 보고한 연구는 6편15,18,20,21,28,30이었으며, 탈락률의 범위는 0.0%~15.6%였다.
중재로 사용된 치료 유형은 한약단독치료 10편16-25, 침치료 6편15,26-30, 뜸치료 5편31-35, 한약과 침의 병용 치료법이 1편36, 한약과 뜸의 병용 치료 1편이다37. 한약단독치료 연구 중 5편은 약재를 환부에 도포하거나 탕전액으로 훈증하는 등의 외용법을 사용16-20하였고, 4편은 경구복용21-24, 1편은 정맥주사치료를 시행25하였으며 침단독치료 연구 중 2편은 온침27,28, 2편은 일반 호침15,26, 1편은 전침29, 1편은 레이저침30을 사용했다. 뜸치료 연구 중 2편은 마사지를 병행34,35하였다. 10편의 한약치료 RCT에서는 복합 한약 추출물을 사용하였으며 단독 추출물을 사용한 연구는 없었다. 중재로 사용된 한약 투여에 대한 부작용을 보고한 논문은 없었다. 본 체계적 고찰에 포함된 RCT중 한방치료와 통상적인 치료가 동시에 사용된 연구는 14편15-17,21-24,26,29,31,32,34,36,37이었으며, 나머지 9편은 한방 단독치료와 통상치료를 비교한 연구18-20,25,27,28,30,33,35였다. 23편의 연구 중 22편에서 치료군이 대조군보다 효율적으로 부종을 제거한다는 결과가 나왔으며 모두 통계적으로 유의하였다. 대조군이 치료군에 비하여 부종 호전에서 높은 효율을 보인 연구는 1편이 있었고, 한약(Wentong Xiaozhong external application)을 외용하는 방식으로 치료한 논문이었으나 연구 결과에 통계적 유의성은 없었다20. 3-arm study 1편에서는 한약과 Mai Zhi Ling pian(General name; Michelle, Ingredient; chestnut extract)을 동시에 복용시켰을 때가 Mai Zhi Ling pian 또는 한약을 단독으로 복용했을 때에 비하여 부종이 의미있게 호전되었고, 한약 단독복용시에는 Mai Zhi Ling pian을 단독 복용에 비해서 부종이 더 많이 호전되기는 하지만 통계적으로 의미는 불충분했다23.

3. 사용된 치료 방법 및 결과

1) 한약치료(n=10)

외용법을 사용한 5편의 연구16-20 중 4편의 연구는 한약 외용제를 사용한 군이 항암치료 단독 대조군 또는 위약치료군과 비교하여 부종의 호전 정도가 높았다16-19. 한약 외용제를 사용한 연구 가운데 1편은 한약 복용과 活血和瘀연고를 외용하는 방식을 치료군에게 동시에 사용하였고, 대조군은 환측상지 온수세척, 림프절 안마와 물리치료를 시행했는데 치료군에서 더 좋은 효과가 있었고 통계적으로 유의하였으나 치료효과가 경구복용 한약에 의한 것인지 외용약에 의한 것인지 명확하지 않다는 점에서 추가적 연구가 필요하다고 생각된다19.
한약 외용법에 대한 5편의 연구 중 나머지 1편의 연구에서는 溫通疎腫外敷方(Natrii Sulfas, Cinnamomum cassia, Rehmannia glutinosa 등)을 환부에 분무하는 방식으로 치료한 군이 플라시보 탕전액을 분무한 대조군에 비해 부종과 상지 운동능력 호전도가 낮게 측정되었으나 통계적 유의성이 없는 결과였다20.
한약을 경구로 복용한 치료군과 일반적 치료만을 받은 대조군을 비교한 4편의 연구에서는 모두 치료군에서 부종과 삶의 질, 부종에 동반된 증상(活動不利, 舌癌, 舌苔黃膩 등)이 더 효과적으로 개선되었다21-24.
한약을 복용시키거나 외용한 것이 아니라 정맥에 직접 주사하는 방식을 사용한 연구에서는 紅花(Carthamus tinctorius) 추출물을 주사제로 만들어 치료군에게 정맥내주사치료를 시행했다. 이 연구에서도 치료군의 환자들이 일반적 치료만 받은 대조군의 환자들에 비하여 부종이 더 효과적으로 개선되었다25.
각 치료마다 투약의 경로는 다르나, 활혈, 이기, 소염의 기능이 있는 약재를 주로 배합하여 림프순환 촉진, 염증 개선이라는 비슷한 효과를 목적으로 했다는 공통점이 있다. 한약치료를 다룬 연구를 정리한 표는 다음과 같다(Table 1).

2) 침치료(n=6)

침치료와 운동치료 등의 일반적 치료를 병행한 군과 일반적 치료만을 시행한 대조군을 비교한 연구는 2편이 있었고, 모두 치료군의 부종과 환부통증이 대조군에 비해 더 크게 감소하였다15,26.
온침을 이용한 2편의 연구는 온침을 시행한 치료군과 디오스민(부종 치료제) 900 mg을 1일 3회복용한 대조군을 비교하였는데, 두 논문 모두 치료군에서 부종이 더 효과적으로 개선되었다27,28.
치료군에게 手厥陰心包經의 혈자리에 전침을 시행한 연구에서는 안전관리, 건강교육, 일반적 동통관리등의 통상적 간호만을 받은 대조군에 비하여 치료군이 부종부위의 피부 탄력, 통증, 무게감 등에서 유의한 호전을 보였다29.
HT1(極泉), LI15(肩髃), LU5(尺澤), PC3(曲泽) 등 총 10가지 혈자리에 헬륨-네온 레이저침을 이용하여 침치료를 한 연구에서도 일반적 치료만을 받은 대조군에 비해 치료군에서 부종이 개선된 비율이 더 높았고 통계적으로도 유의하여, 호침 뿐 아니라 레이저 침도 의미있는 치료가 될 수 있다는 근거를 보여준다30.

3) 뜸치료(n=5)

뜸치료와 일반적 치료를 병행한 군과 일반적 치료만 받은 대조군을 비교한 연구 2편에서는 모두 치료군의 부종이 대조군에 비해 더 잘 회복되었다31,32.
치료군에게 상지 아시혈 위주의 뜸치료를 시행한 연구에서 역시 pneumatic circulation을 받은 대조군에 비해 상지 수평 둘레의 감소율이 더 높았으며, 환측 상지의 동통도 더 적었다. Pneumatic circulation은 공기의 압력을 이용하여 신체의 특정 부위를 조였다 풀면서 마사지를 해주는 치료 기기이다33.
뜸치료와 마사지를 병용한 연구는 총 2편이 있었다. 뜸치료와 혈자리 지압을 함께 받은 치료군에서 일반적 치료만 받은 대조군에 비하여 부종 뿐 아니라 환측 어깨의 기능이 효과적으로 개선되었다34,35.

4) 침치료+한약외용법 병용(n=1)

침치료와 한약 외용법을 동시에 사용한 연구는 1편이었다. 침치료를 하고 加味芩黃膏라는 한방 외용제를 환부에 도포한 치료군의 상지 둘레가 물리치료만 받은 대조군에 비해 더 많이 줄어들었고 환측 어깨의 활동 범위도 더 커졌다36.

5) 뜸치료+한약외용법 병용(n=1)

뜸과 한약치료를 함께 시행한 연구 1편은 뜸치료와 동시에 한약으로 환부를 훈증하고 목욕하는 방법을 중재로 사용했다. 치료군과 대조군에게 공통된 치료로 단삼추출물을 정맥주사하였으며 상지 기능운동을 실시했다. 건측과 환측의 상지 둘레를 측정하여 결과를 비교했을 때 치료군이 대조군에 비해 건측상지와 환측 상지의 둘레차이가 적어 부종이 효과적으로 개선되었음을 시사하였다37.

4. 비뚤림 위험 평가

1) 무작위 배정 순서 생성

5편의 RCT15,17,24,27,33는 난수표를 이용한 무작위 배정 방법을 기술하여 비뚤림 위험이 낮았으며, 나머지 연구는 배정 방법을 기술하지 않아 ‘불확실’로 판정하였다.

2) 배정 순서 은폐

2편의 연구15,27를 제외한 나머지는 배정순서 은폐에 대한 명확한 언급이 없어 ‘불확실’로 판정하였다.

3) 연구 참여자, 연구자에 대한 눈가림

2편의 연구20,36에서 대조군에 플라시보 한약을 사용해 비뚤림 위험을 ‘낮음’으로 판정하였다. 반면, 나머지 연구들은 눈가림에 관한 언급이 없었다. 그 중 1편의 연구18는 치료군과 대조군의 치료방법은 유사하나 외용 또는 복용한 용액의 색과 향에 차이가 있어 눈가림이 확실치 않았을 것이라 생각되어 ‘불확실’로 판정하였다. 다른 19편의 연구는 대조군의 치료법이 상이하여 연구 참여자 및 연구자에 대한 눈가림이 이루어지지 않았다고 판단하여 ‘높음’으로 판정하였다.

4) 결과 평가에 대한 눈가림

본 고찰에 포함된 모든 RCT는 결과 평가에 대한 눈가림을 언급하지 않았다.

5) 불충분한 결과 자료

6편의 연구15,18,20,21,28,30는 탈락률과 탈락의 이유를 명확히 보고하여 비뚤림 위험 ‘낮음’으로 판정하였고, 나머지 연구는 탈락률과 그 이유를 서술하지 않아 ‘불확실’로 판정하였다.

6) 선택적 보고

결과 평가의 객관적인 기준을 정해두지 않은 1편의 연구32를 제외한 나머지는 사전에 평가기준을 밝히고 그에 따른 결과수치를 모두 보고하여 비뚤림위험 ‘낮음’으로 판정하였다.

7) 그 외 비뚤림

그 외 비뚤림으로 기준선의 불균형, 데이터의 이질성 및 오염 여부, 이해의 상충 등을 고려하였으나, 특이한 비뚤림이 관찰되지 않았다(Fig. 2, 3).

IV. 고찰 및 결론

본 연구는 유방암 환자의 상지 림프부종 및 삶의 질에 대한 한의학적 치료의 임상적 효과를 입증하기 위해 림프부종을 갖고 있는 유방암 환자를 대상으로 한 국내외의 연구를 종합하여 체계적 고찰을 시행한 것이다.
고찰한 연구들 중 가장 많은 수를 차지하는 것은 한약을 이용한 치료였으며 경구 복용 뿐 아니라 외용법을 다룬 연구도 많았다. 주로 사용된 약재는 桃仁(Prunus persica), 紅花(Carthamus tinctorius), 水蛭(Hirudo nipponia), 當歸(Angelica sinensis) 등 活血去瘀藥과 川芎(Cnidium officinale), 木香(Aucklandia lappa) 등 理氣藥, 金銀花(Lonicera japonica), 蓮翹(Forsythia viridissima) 등 凊熱解毒藥이었다. 가장 많이 쓰인 약재는 6편의 연구17,19-21,23,36에서 언급된 當歸(Angelica sinensis)이며, 利水退腫, 活血和瘀, 行氣活血 등의 목적을 가지고 君藥 또는 臣藥으로 처방에 배합되어 림프부종 개선에 주요한 역할을 하였다.
침치료로는 온침27,28과 전침29을 이용한 연구, 레이저침30을 이용한 연구도 있었는데 모두 일반 호침과 마찬가지로 치료군이 대조군에 비해 부종이 더 잘 호전됨을 알 수 있었다.
뜸치료 역시 부종 호전에 효과적이다. 특히 한 연구는 항암요법을 3차례 받는 동안 뜸치료를 시행하여 이후 발생한 부종의 심한 정도를 비교하였는데, 치료군이 대조군보다 경도 부종 발생 비율이 높고, 대조군은 중등도 이상의 부종이 발생한 비율이 더 높아, 뜸치료가 부종 발생의 예방에도 효과적이라는 사실을 보여준다31.
침, 뜸치료에 가장 자주 사용된 경혈은 7편의 연구에서 언급된 手厥陰心包經이다24-26,29-32.
이 고찰에 포함된 RCT 중 대조군이 치료군에 비해 오히려 더 나은 결과를 보인 연구는 1편이 있었으나 이는 통계적으로 의미가 부족하였다(p>0.05).
본 연구가 지닌 한계점은 먼저, 한의학적 치료의 범주를 한약, 침, 뜸으로 제한하였다는 점이다. 둘째, 본 고찰에 포함된 23편의 RCT는 연구 참여자와 연구자에 대한 눈가림이 제대로 이루어지지 않은 논문이 대부분으로 연구방법의 질이 높지 않은 편이다. 또한 이 고찰은 4개의 DB에서 검색된 한국어, 영어, 중국어의 RCT를 포함하므로 본 고찰에 포함되지 않은 추가 문헌이 있을 수도 있다.
그러나 본 연구는 한의학적 치료가 기존의 림프부종 치료에 비하여 유방암 환자의 상지 림프부종과 운동능력을 더 효율적으로 호전 시킬 수 있다는 근거를 얻기 위해 다양한 방식의 연구 결과를 종합했다는 의의가 있다.
결론적으로 이 체계적 문헌고찰은 침, 뜸, 한약을 포함한 한의학적 치료가 유방암 환자의 림프부종 개선에 잠재성이 있으며 향후 유방암 환자의 림프부종 치료에 한의학적 치료가 활용될 수 있음을 제시한다.
